1/3/21 - ANYONE ENTERING A HEALTH CENTRE BUILDING/GP SURGERY IS REQUIRED TO WEAR A FLUID RESISTANT SURGICAL MASK UNLESS THEY ARE EXEMPT. THESE ARE PROVIDED AT THE SURGERY IF YOU DO NOT HAVE ONE. THANK YOU FOR YOUR CO-OPERATION.
Repeat prescriptions are issued at the doctors’ discretion and are normally for patients stabilised on long term treatment. You should have been given a reorder form. Please fill this in carefully by marking the box next to the medicines that you need.
Please ensure your chemists name is on the form. Medicines not on your reorder form must be added and indicated with a tick if you require this item.
Hand or send in the form to the health centre or chemist at least 4 days before you need them, excluding weekends and public holidays.
Alternatively you can order online via the link at the top of this page. New users are required to complete a simple registration prior to using this service.
You are also now able to phone your repeat prescription request to our dedicated telephone line - 01356 624411 and choose option 3 if you prefer. We regret that, in the interests of safety, we no longer accept requests for repeat prescriptions over the telephone.
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
Please allow 4 days, excluding weekends and Bank Holidays, for your request to be processed. Any problems please telephone the surgery.
